Kinetica 1's third stage lost attitude control seconds after ignition and was terminated by the range safety system. A power supply wire to the stage's thrust vector control mechanism had been damaged during installation and failed under vibration during powered flight; the first two stages performed nominally.

This was the vehicle's first launch failure - all five prior missions had succeeded.
