Hitomi

H-IIA carried Hitomi, JAXA's X-ray astronomy satellite, to orbit along with three small satellites. About five weeks later, on 26 March 2016, controllers lost contact after a faulty reading from an attitude sensor led the satellite to spin up. JAXA concluded the solar panels had broken off and ended recovery efforts on 28 April.
