DEC 22202501:51:30 UTCH3 lost QZS-5 when the second stage's second burn shut down prematurely, leaving the navigation satellite short of its planned Tundra orbit. It was the rocket's second launch failure; the first flight, in March 2023, ended when the second stage failed to ignite and ALOS-3 was lost.
