SEP 03201607:00:00 UTCFalcon 9 and AMOS-6 were destroyed on 1 September 2016 in a propellant-loading fire during a routine static fire test — two days before the planned launch. The explosion originated near the second-stage LOX tank; SpaceX's investigation concluded the root cause was a buckled liner inside several composite-overwrapped helium pressure vessels, allowing super-chilled liquid or solid oxygen to accumulate beneath the liner where it ignited from friction. No personnel were on the pad. Following the accident, SpaceX suspended static fire tests with the payload installed.
