Epsilon-6 failed when four of eight attitude control thrusters on the second stage did not activate during a coast phase, causing the rocket's attitude to deviate 21 degrees from the planned trajectory. JAXA issued a flight termination command, destroying the vehicle and all eight payloads.

The failure was Japan's first orbital launch vehicle loss in nearly 20 years, and the first Epsilon flight to carry commercial payloads, among them iQPS's first two operational radar imaging satellites.
