The first failure of Sea Launch, on the venture's third flight from its equatorial ocean platform. A ground software fault left a pneumatic valve in the Zenit's second stage open before liftoff; helium bled away, the second-stage engine shut down early, and the ICO F1 mobile-communications satellite never reached orbit.
