Telstar 14

One of the satellite's two solar wings failed to deploy fully after separation, cutting its available power and forcing several transponders offline. The defect proved to be a design flaw: the replacement spacecraft, Telstar 14R, suffered the same stuck-array failure when it launched in 2011.
