Ofeq-6

The Shavit's third-stage motor failed to ignite, and the Ofeq-6 reconnaissance satellite fell into the Mediterranean. The loss set back Israel's space-based imaging program, which had to keep the aging Ofeq-5 in service until a replacement reached orbit in 2007.
